PART 2:  We need some contact details for the attendee? These will only be used if we need to contact you about this survey.

PART 3:  Does the attendee qualify for free meals? There is no charge for mental health service users. 


Accompanying carers / support staff, friends or family members (who are not service users) may be asked to donate £10.00+ toward the cost of their own meal.

PART 4:  Which of the following meal dates would work for you? You can select as many as you wish!


Please Note: Meal sessions will normally be in the evening (from approx 7:00pm) and some may be held on Saturdays.

SECTION 5:  Support with travel needs!


For attendees experiencing financial difficulties our Hardship Fund may be able to contribute towards local travel costs.

SECTION 6:  Please Note: Answers to this question will help us identify restaurants that will best meet the needs of our attendees.


Each restaurant will have their own guidance of dietary needs and allergies as required by law.
